The focus of the programme is to offer relief services to people and families in underserved rural communities in and around the crossroads of the Mpumalanga, Gauteng and Free State Provinces. This area, often referred to as a forgotten part of the country due to its scant infrastructure and lack of large-scale industry, is home to several large rural communities situated around the towns of Balfour, Grootvlei, Villiers, Greylingstad and Cornelia. The Topsy Foundation is the only organisation of its kind with an active presence in these communities.

Other than the larger communities where Topsy has an active presence, relief services are also offered to people from other communities, including Tweeling, Cornelia, Frankfort, Heidelberg and Johannesburg.
